FILE A CLAIM

Pursuant to the Court's Order Preliminarily Approving Settlement and Providing for Notice, entered on May 26, 2022, and the Joint Statement and Stipulation and Order Modifying Settlement Schedule entered on June 8, 2022, Class Members who wish to participate in the Settlement shall complete and submit the Proof of Claim and Release form ("Proof of Claim") in accordance with the instructions contained therein. To obtain a copy of the Proof of Claim, click here. The deadline to submit a timely Proof of Claim is September 13, 2022.

If you fail to execute and submit a timely, properly completed and addressed Proof of Claim, your claim may be rejected and you may be precluded from any recovery from the Net Settlement Fund.
